Expert Skilled Nursing Care for Recovery and Beyond

Subacute Rehabilitation

A level of care designed for patients who are medically stable but require intensive therapy and nursing care following hospitalization. It serves as a bridge between the hospital and home, focusing on recovery from surgery, illness, or injury. Subacute rehab typically includes physical therapy, occupational therapy, and sometimes speech therapy, aimed at restoring independence and improving quality of life. These programs are offered in our skilled nursing unit, providing 24/7 care and monitoring to support a safe recovery.

Post-acute Orthopedic Care

A specialized care provided after surgery or hospitalization for orthopedic conditions, such as joint replacements, fractures, or spinal surgeries. It focuses on rehabilitation, pain management, and recovery to restore mobility and strength. This care often includes physical and occupational therapy, wound care, and tailored support to help patients regain independence and safely transition back to daily activities.

Stroke Recovery

Focuses on helping patients regain function and independence after a stroke. It involves a multidisciplinary approach that includes physical therapy to improve mobility, occupational therapy to rebuild daily living skills, and speech therapy to address communication or swallowing difficulties. Care teams provide medical oversight, manage secondary complications, and offer emotional support. The goal is to maximize recovery, promote neurological healing, and support a smooth transition back to home or a senior living community.

Cardiac Care
